During your first appointment and consultation with Dr. Tad or Dr. Anderson explain your diagnosis and treatment plan options. Occasionally, treatment can be done the same day.
If you have a complex medical history or treatment plan, you’ll need a second appointment for us to provide you with the best treatment.
Please provide the following information at your first appointment:
- Completed New Patient Forms.
- A list of medications you are currently taking.
- Any medical or dental insurance cards. This will help us file your insurance claims.
Please note: A parent or guardian must accompany patients under 18 during their first visit to our office.
Please let us know if you have a medical condition that may be of concern prior to your appointment (i.e., diabetes, high blood pressure, artificial heart valves and joints, breast implants, rheumatic fever, allergies, etc.) or if you’re on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e., antibiotics for pre-med.).
